Psychological Consultants accepts referrals of new clients from all sources. Customers requesting information regarding admission to PCM are asked to contact our Intake Specialist at 269-968-2811 or 800-261-8850, extension 40. Questions regarding services and referrals may also be sent by e-mail using the comment box below. New referrals are processed in person or by telephone. Information regarding the client, the nature of the referral and requested services will be requested. The Intake Specialist will schedule the initial intake session and provide instructions regarding this first appointment.

In many instances pre-approval for services is required by insurance, managed care and/or HMO companies. Customers are encouraged to initially contact their insurance carrier to clarify benefits and receive necessary pre-authorizations.

Prior to the first appointment new clients are expected to complete written forms which provide basic information regarding the client and their family, the client's health history, mental health and substance use history as well as health insurance. Written releases of information will be requested to allow PCM to communicate with the client's referral source, primary care physician and health insurance carrier. A legal parent or guardian is necessary to complete requested paperwork for child and adolescent clients (less than 18 years of age). Customers will be requested to arrive 30 to 60 minutes prior to their initial appointment to complete necessary written documents and meet with billing staff. For the convenience of new customers written forms are available below for children/adolescents (0 to 17 years of age) and adults (18 years and above).

The time required prior to the first appointment will be reduced when admission forms are printed, completed and brought to PCM for the initial visit. Please select the appropriate forms below based on the age of the client.
